Within Gravity Forms:

  • On the right side of the screen choose -> Advanced Fields
  • Click Name & Email to add to form
  • Edit Name Field:
    • Edit Format -> Choose:  Simple
    • Under Advanced Tab
      • Field Size -> Select:  Large
      • Input Default Value (Ex. Name)
      • Input CSS Class Name “gforms-placeholder”
  • Edit Email Field:
    • Under Advanced Tab
      • Field Size -> Select:  Large
      • Input Default Value (Ex. Email)
      • Input CSS Class Name “gforms-placeholder”
  • Save

Note: This form is then applied to a widget in Appearance.
